CAF in Action: Operationalizing a Community Accountability Framework in India
This guide is designed to help expand the role of TB-affected communities to provide bottom-up and community-led feedback to the health system. The aim is to improve the Quality of Care and Quality of Services for people and communities affected by TB. The CAF model was designed to enhance the accountability, coverage, and effectiveness of the TB programme and generate demand by strengthening community confidence in TB services.

The Accountability Leadership by Local Communities for Inclusive, enabling services (ALLIES) Project strives to end TB through community initiatives. The goal of the USAID supported project (October 2019 to September 2023) is to enable the environment for TB elimination by leveraging community action as an ally to build a culture of accountability.
The ALLIES project is being implemented in three districts each in Chhattisgarh, Jharkhand and Odisha and in six districts in Tamilnadu.

Integrating Empowerment Evaluation with the Community Accountability Framework (slides).

Empowerment Evaluation and the Community Accountability Framework (CAF) are grassroots, community-based approaches to improving health.
The American Evaluation Association presentation highlighted efforts to integrate these approaches, recognizing any application is dependent on the local context and conditions.
A combination of integrated efforts were discussed including apply empowerment evaluation first to establish the community’s holistic perspective, including a host of concerns such as education, housing, employment, and health. This would establish a larger framework in which to address specific TB concerns using CAF, which focuses on TB related questions such as:
1. How many days after testing were you given your diagnosis?
2. Was your weight taken at the TU when you started treatment?
3. At the time you were diagnosed with lung TB, were you and your family counseled on cough hygiene?
This macro to micro approach could be adopted if TB was at a manageable level or rate.
In contrast, a micro to macro-orientation was discussed. In villages where TB is rampant, there is no time to look at the “big picture”. It is important to focus on the immediate needs, such as identification, treatment, and prophylactic practices. In this context, empowerment evaluation exercises would follow the CAF focused approach.
A meta approach was also discussed in which empowerment evaluation is used to evaluate the CAF in practice, to refine, reinforce, and improve the approach in the community.
The epiphany of the session was an agreement that there was no single best approach concerning the integration of approaches. The combination or integration of approaches is dependent on local conditions, customs, and concerns.
